Brief Answer:
As discussed in details below.

Detailed Answer:
Hi.
Thanks for your query about your friend.
To recapitulate: Male/57 - blood leaching out from stool in toilet - stool itself is not red - No visit to a Doctor - no medications ...
We can say that the blood is in streaks on the stool.
The commonest causes with explanation are:
- Hemorrhoids also called as piles can lead to blood in streaks on stool, usually painless unless complicated by thrombosis or prolapse.
- Fissure in ano - always painful passage of stool with blood on stool.
- Polyp/Mass can also give similar things although rare.
The best way to get the proper diagnosis is as follows:
Per-rectal digital examination and ano-proctoscopy, preferably by a General Surgeon is the only sure way to get the correct diagnosis and thus to get a plan of management.

I hope this answers your query.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
