Question : I have a history of kidney stones and I also have interstitial cystitis. I am currently passing clots and blood in my urine and feel like I have to constantly go to the point of it being very uncomfortable. Should I go to the er now or wait it out until morning and go to walk in clinic? Also I woke up with abnormally swollen fingers yesterday. Didn't happen again this morning

Plan for hematuria:

Detailed Answer:
Hello XXXXXXX

For an uncomplicated lower (bladder only) urinary tract infection, you could probably wait and increase the amount of water you are drinking.

But with passing blood clots, it would be best to get checked out. If there are any all night urgent care clinics near you, I recommend going there, but if that isn't available, then do go in to the ER.

In the clinic or ER, they should do a full urinalysis and if there is indication of infection, make sure that they send the specimen for culture and sensitivity testing. As you may know, given your history, a sensitivity test shows which antibiotics are effective against the particular bacteria you may have (if the culture is positive).

The bleeding could be from a bad bladder infection. But with your history of stones, that is a possibility too. As are other problems occurring in the kidneys, but the risk of those problems (autoimmune inflammation, tumor) are much less likely. With autoimmune problems, usually there are fevers and feeling overall ill, for example.

I am not sure what to make of the swollen fingers yesterday, unless perhaps you used your hands more or ate more salt. But it would be reasonable, given this plus the gross hematuria you are noting, to have a blood test for BUN and Creatinine which are kidney function blood tests.

I would welcome, if you are willing, for you to let me know how you are doing after you have gone in to be evaluated.

I hope this information helps. Please let me know if I can provide further information.

I went to the er last night but ended up giving up and coming home because I was more comfortable here than in the waiting room. Went to clinic this morning. It's definitely a UTI and they said more than likely my interstitial cystitis flaring up. Thank you so much for your help!
