I have gone through your query in detail, and understand your concern.
Blood in urine, especially painless gross hematurea in this age group needs evaluation to rule out any neoplastic lesions of the urinary tract.
The fact that urine cleared out on starting antibiotic alone can not suggest the diagnosis to be UTI. This need further evaluation. If the CT we are going to do shows no focal lesions, we can be confident. Further diagnostic procedures like cystoscopy is needed only if the hematurea recurs. Your doctor will guide you.
Anyhow, most of such cases are caused by UTI or benign lesions like cysts.
