The stain was in both front and back of his underwear. He hasn't noticed anything else. No sex lately. A little sore on one of his testicles is all.

Brief Answer:
He'll need to get examined by a doctor for the sore testis.

Detailed Answer:
Hello XXXX,

In case the sore on testis was noticed,it could be bleeding from the scrotal skin also.
There may be no particular cause for it,but it's safest to get it examined by a doctor.
Such cases of bleeding from the scrotal skin, are known to recur,at any time.

Before seeing the doctor, get the following tests,so it's easier to find the cause.
1. urine routine.
2. blood routine,RBS,LFT,creatinine,PSA,platelets,and,BT,CT.and PT.
3. stool routine.
4. semen routine.

According to the reports, the doctor after examination, will be able to advise the
proper treatment.
If you want an expert opinion on the reports, you're welcome to send it here.

Wishing him good health.
