Question: My wife (age 72) had visual problems today, similar to migraines she's had before (no pain). But she also had problems thinking (how to unlock the car door) and remembering words.

Visual problems can occur during migraine attacks and rarely impaired mental activity.
At this age, in absence of headache, possibility of stroke should also be kept for such a presentation.
You should get her examined in an ER and get a CT head done.
