Detailed Answer:
Hi and thanks for the query,

The symptoms you describe are very compatible with an infectious process, either viral or bacterial. It might be necessary to know if you have had any upper respiratory symptoms like running or stuffy nostrils, coryza or not. If yes, the possibility for this to be a viral infection is greater. If, a bacterial infection needs to be carefully checked.

Tylenol would be sufficient in case the lung exam is reassuring and a bacterial infection excluded. Rales on lung exam, chest X ray, and failure to respond to Tylenol could indications that it might not be of viral, but of bacterial origin, and would require some antibiotics. It is not however rare to experience persistence of symptoms after use of Tylenol, not indicating inefficiency. patience and careful watching of patient and symptoms are at times needed.
A complete blood count should show raised white cell counts, predominantly lymphocytes in case its a viral infection, and neutrophils in case of a bacterial infection. A creactive proteins, CrP, will be raised in case of bacterial infection. These tests are easy to do and to decide on the treatment options to make, Viral infection treatment would be symptomatic (pain and fever control), while in case its a bacterial infection, antibiotics might be required.

It might be necessary to screen for some common viral infections that present as such like HIV, infectious mononucleosis. In case of bacterial infection, added to complete blood counts and CrP tests, a blood culture might be needed.

A complete clinical review by your doctor and guided blood tests are required to guide diagnosis and management. Thanks and kind regards as I wish you the best of health.
