Question : This is about my sister in Poland. In XXXXXXX she weighed approximately 160 pounds. It was at this time she would eat her regular meals, then ten minutes later she has very heaving coughing which causes her to throw up all the food she has eaten. She then feels faint and she has to lie down for approximately an hour. When she gets up she does not want to eat anything. She is basically living on water and tea. She has lost 50 pounds in 3 months. The Doctors in Poland have not been able to diagnose her problem. She continues to lose weight every week. She spends most of her time in bed as she is feeling very weak.

I appreciate your concern

The top two differentials that are possible with this presentation are Tuberculosis and malignancy. Sputum for AFB, chest x ray, blood counts, ESR, CRP should be done. X rays alone wont be enough to exclude malignancy. In the presence of 3 months history of weight loss associated with cough or fever and night sweats can typically occur in tuberculosis and would be the top hunch if the blood counts, peripheral smear and bone marrow examination come out to be normal. Consult an infectious disease or internal medicine specialist in this regard for further management.

This just came on all of a sudden. Can TB be cured? I had a feeling it might be that with the terrible cough. Also should she be drinking a drink that replenishes electrolytes? The medical service in Poland is not that good so I am very worried.
