What Causes Breast Inflammation During Lactation?

Question: Dear Sir,
Good Morning!
The case being referred to you for advice is for my Wife. Post her third pregnancy (all normal deliveries), many at times she was diagnosed for breast inflammation. This is her eleventh month of lactation. Inflammation in the sense - redness and hardness. When it was first reported to a Breast Surgeon, she was advised for USG - which was done in the month of Nov-13. Both the reports are attached (USG_Old 1 & 2). Again on 30-May-2014, we had met the Surgeon after having noticed pus discharge from one of the milk duct in the right breast. Doctor examined and observed that it has subsidised and advised to feed the baby as well. She didn't notice any inflammation on the breast. She has not advised for any medication - but suggested to feed the baby in full.
Post that (after 4-5 days), my wife felt pain on the right breast associated with temperature around 102-103 degrees and was immediately referred to the surgeon. She again advised for USG and the same was done on 04-Jun-2014. Pus was noted and the same was removed by Aspiration on the same day. Sample Pus was sent to Lab and the report is also attached. Post the procedure, Surgeon had prescribed for an antibiotic (Ceftum 500 mg - twice daily for 5 days), Enzoflam and Tab Pan 40 mg. She had completed this course without Enzoflam.
Post the Aspiration was done, the very next day, my wife observed blood-stained discharge through the nipple - from one duct. We had contacted the Surgeon and she said the same shall get subsidised in a couple of days. Since it has not subsidised, we had met the doctor yesterday (09-Jun-2014) who has done the Aspiration procedure. The doctor assumed it could be due to use of pump or rare chances due to the prick of needle and advised us to meet the Surgeon again who shall advise further treatment. We shall be meeting Surgeon today.
Would request you to advise us on the findings of the USG, examining the reports in detail and the aftermath she is experiencing after Aspiration - very particularly on the blood-stained discharge which is still continuing.

I have read your query and understand your concern.

To me it looks like a case of breast abscess and mostly during the needle aspiration a blood vessel has got injured to cause the bleeding or the infection has eroded a vessel wall. And milk is hampering the formation if blood clot.

In either case it should stop by itself. You should had given her enzoflam as it is antiinflammatory and would had greatly helped in early healing.

Another option is that, as this is her eleventh month of laction so her lactation can be stopped with drugs and this will help in better control of infection and bleeding as after stopping lactation she can be prescribed better drugs for the same without any harm to the baby.

As today you are planning to consult your doctor, you can discuss these aspects also with him.

As per the pus culture, there is no infection there only inflammation and reactionary fluid is there and that should get controlled with your enzoflam.
