What Causes Brown Spots And Slight Irritation Around Genitals After Intercourse?

Question: Hello Doctor.
So I have a question that I'd like to ask you. I recently had protected sex with a woman twice last week. Our first encounter involved just her performing oral sex on me with a condom on.

Our next encounter was about four days later and this time we had regular (i.e., vaginal) sex. We both came twice and I wore and replaced the condom after the first time I came. About a day or so later, I noticed a straight horizontal line of faint, brown dots about 1mm or so in size on my glans and I could not help but wonder if this was a symptom of something or if it had been there long ago. A day ago, I also started feeling a very slight irritation in the same area and again I started asking the same questions. The dots are still there, but they seem to have faded just a bit.

Our sex was fun and a bit rough and so I am hoping that all this may have been due to friction or something along those lines. In case, any help would be much appreciated. Thanks.

I should also like to add I am male, in my late 20s, that the dots are flat, and this was my first time at being sexually active.

Hello. Thank you for writing to us

I have read your query and I have noted down your concern.

A condom protected sex is relatively safe as far as risk of an STD is concerned. I don't think it is an STD.
Moreover the description does nt seem to match any STD in particular. The only possibility that i can think of is genital Hpv infection which can sometimes present as flat, brown Or skin colored, slightly raised bumps.
However since you have noticed these brown spots on glans which normally would have been covered by condom therefore it is unlikely that these are warts.
Anyways i would have preferred an Image so that I could have had a look at them. You can still do that by uploading an image in Reports Section.
Most likely they seem to be either due to friction Or may be due to dermatitis to condom.
Having said that, a condom protected sex is not completely safe. STDs like genital herpes and HPV can still be transmitted from infected sexual partner. This is because a condom cives only a portion of shaft of penis i.e glans and neighboring shaft and it leaves base of shaft and adjacent pubic region exposed for skin to skin contact during sexual contact.

Dear Doctor,

Thank you for your response. That certainly clarifies matters. I have uploaded an image, whose quality is not so great, but hopefully may give you some idea on what I am talking about. As I said in my initial question, I am feeling just a bit of a sensation at the top, where it is a little red.

Is there anything I can do to help relieve it?

Hi.

The image is not clear and it does'nt help. I request you to upload a better image which is clear and allows me to give you a sound advice.
Though it seems to me that the brown spots are probably mucosal lentigines which are benign and may have been there before this sexual encounter. They don't require any intervention.
I can make out some redness on glans which is probably due to friction or contact dermatitis to latex in a condom. You may try a moderately potent topical steroid e.g triamcinolone acetonide 0.1% cream for the red areas.
Anyways, I would suggest you to get tested for genital herpes (the reason for which I gave you in previous reply). An ELISA for Hsv type 1& 2 (IgG) at 12 weeks would be confirmatory.

Dear Doctor,

I tried uploading better quality photos, for which see attached. The irritation has died down somewhat after taking a shower. I should also like to note that it must have grown more agitated after yesterday, when I masturbated for a particularly longer time than usual.

Thank you again for your responses. I guess the only lingering question I have is should I be worried of having contracted any other diseases? As I said this is my first time I in all this and so I'm extremely paranoid at everything.

Thank you. The image is much clear and everything looks ok except for mild redness which is probably due to friction. This would subside on its own and I would not suggest anything for it.
As I already mentioned, Hpv and genital herpes can still be acquired despite using a protection though the risk is much less with a condom than without a condom. Since Hsv can be asymptomatic and therefore I suggest that you take a screening blood test for the same i.e ELISA for Hsv type 1& type 2 (IgG) at 12 weeks.
Unfortunately there is no approved screening test for Hpv in males.

As I said above, I am particularly paranoid at this point and am keeping a careful eye out on everything. Today would mark the one-week point of our encounter, and just a few hours ago I noticed a slight crack on the skin under the upper part of my lip. I don't know - the weather has been a little colder than usual the last few days. Is this, too, benign? I did not have any skin-on-skin oral contact with the woman in question (except a few kisses planted on her neck). But is this something new I should worry about or am I getting worked up for nothing?

Also, at this point what should I keep an eye out on or what steps should I take in terms of hygiene and other matters? Is there any other advice you can impart?

I think it is just chapping of lips.
You cannot get herpes on lips if your did'nt had any sort of direct contact e.g a kiss or oral sex on her.
You can use a lip balm and that should be fine.
