Brief Answer:
Need a few tests to answer your questions

Detailed Answer:
Hi

I have gone through your history. It will be difficult to answer your questions without a few tests. Since you are having brown urine with a musty smell I would advise you to do a urine microscopy culture test. It is also advisable to do a fresh liver function blood test along with a CT scan of upper abdomen. Your treating doctor will know about these tests and can guide you. Please discuss about it with him/her.

Reports of aforementioned test will provide clues to the cause of brown musty urine.

Let me know if you need any more information.
Regards
