Seborrhoic dematitis with multiple abscesses.
Fistula formation
Affection by odd but specific bacteria causing this problem.

I would advise you the the following::
Clean the area regularly with liquid povidone iodine.
Consult a General Surgeon, have a physical examination to have a clinical diagnosis, so that further steps can be undertaken to have a proper and definite diagnosis.
You may need a biopsy. This is most wonderful tool to have a confirmed diagnosis.
Always remember- Once the diagnosis is perfect , then only it is possible to have a proper treatment. I hope you understand what I mean by this.

Thanks for the pictures and the history you have provided, I have gone through the pictures.

Hair can be seen through the lesions, some picture shows scar tissue, some shows the typical picture of chronic appearance.

This is some sort of chronic dermatitis with super-added infection. This is recurrent.

I would advise you the following::
Apply Liquid Povidone Iodine available as Betadine Solution (do not buy the Betadine Scrub) to the affected area in the morning once you get up, and after the bath, it dries nicely.
Avoid Hot and humid places (difficult in XXXXXXX and in XXXXXXX area in particular, I know).
Use loose underwear and pants.
Let the area be aerated whenever possible.

I would like you to take an Opinion of a Renowned Dermatologist in XXXXXXX (I hope it is not far way) before starting any antibiotics and do not apply any local ointments till then. This will ensure that he can see the exact condition as Dermatology (Skin) diagnosis depends upon the visual impression.

He may then start a proper course on clinical judgement or can advise biopsy if he feels so.
