What Causes Bumps On Scalp Along With Itchy Scalp?

1. Two small bumps have formed on my scalp at two separate locations since past year. They do not itch, hurt, harden, or have pus. But they are not healing.
2. My scalp is always itchy and a bid red in places. Sometimes small pus points form on the scalp, which later dry or burst. Hair around these points fall quickly.
3. Despite washing my hair regularly, big flakes of dandruff always form on my scalp.
4. I have excessive hair fall and I am losing hair from the top part of my scalp.
I sought medical consultation for this condition about 1.5 years back. I was prescribed Cebowash shampoo by the dermatologist to wash my hair with. I am continuously using this shampoo since then.
I wash my hair once a week, as washing twice only increased the hair fall during and after the wash.
I oil my hair with coconut oil (Parachute). If scalp is too itchy, I apply Himalaya anti-dandruff cream on the scalp. This cream soothes the itching, but only for a while.
I touch up my hair with XXXXXXX once a month (Richfeel of Shehnaz Herbal branded henna). I have been continuously doing so since past 5 years.
Please help identify the medication and other steps to heal my scalp and reduce the hair fall.
