Detailed Answer:
Hello,XXXXXX,
Welcome to HCM.
I am Dr Rakesh Sharma answering your query.
I can understand your concern. But there is nothing to worry about it.
It may be mild abrasion caused by stools or wipes and washes.
If I were his treating doctor I would have advised PLAIN silver sulfadiazine cream,or if not available , any SOOTHING cream or lotion.Make sure they are nonirritating.
He would be feeling better within no time.
Hope I have answered your query, if any thing else, you are welcome or you can close the question by rating the answer.
Good wishes.
