What Causes Burning Lip Pains?

Question: Burning lip pains. Tried everything and no relief. Have seen two Dermatologist, Rheumatologist and Dr. of Internal Medicine. XXXXXXX will be one year.

Dear Friend.
Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
I suppose when you have been to many doctors they must have investigated and examined you to rule out all possible causes.

But, if they are unable to find any cause and trusting there capabilities, i would say you might be suffering from "Burning Mouth Syndrome (BMS)", which is a painful, condition often described as a scalding sensation in the tongue, lips, palate, or throughout the mouth.

However, before i give you that diagnosis strongly, answer the following:

1. Since how long have you been suffering from this?
2. How did it started?
3. What investigations have been done? Attach reports.

Also tell me into which of the following three you fit into:

1.Symptoms not present upon waking, and then increase throughout the day
2.Symptoms upon waking and through the day
3.No regular pattern of symptoms

Came on with no warning in Jun 2013
No idea how it stated.
Both Dermatologist (2) performed culture, biopsy, valtrex, predisone, mouth washes, lip ointments, and discontinued various Rx drugs for a period of time; then re-continued. All to no avail. I still suffer. Recently, Physician discontued all medication but Diovan HCT and is considering Denavir.
I await your reply. XXXXXXX

BMS is a possibility.

Dear Friend.
If there are no visible sores, Denavir is unlikely to help.

The diagnosis that i suggested BMS, is a diagnosis of exclusion.

So, i would advise you to get following tests (In case if you have all, please attach the reports):
1. Thyroid Profile
2. Hemogram
3. Vitamin B12 And Folate Assays.
4. Blood sugar levels.

If you have any lesion over there or sore tongue, let me know, then it can be Lichen Planus.
If examination or Oral Cavity is absolutely normal, then Dermatological Disease is Unlikely.

I would like to know about these investigations and also want to see biopsy report.

Review with reports.

I will follow-up with my primary care Physician as to your recommended test. Would the test performed by the Rheumatologist indicate any problems relative to your recommendation. The only thing mentioned was slight anemia but everything else indicated no problems; primary Physician said to take a Iron supplement. It appears to be a mystery to all concerns. I do have two (2) lesions on the inside of the mouth/cheek and a slight sore on the lower lip, probably from bleeding. The burning pain is unbearable.

Dear Friend.
This BMS is a diagnosis of exclusion. It won't come in any test. So, if all the tests are normal and examination is normal, we suspect this syndrome.
1-2 oral ulcers won't cause such severe burning.
You can take treatment for those ulcers. If your burning problem persists after they resolve then, i feel my diagnosis get upper.
Discuss with your doctor my opinion.
Review with me once your ulcers have healed.
