Thanks for the additional information.
Based on your description , a cardiac cause should be ruled out primarily especially in the presence of hypertension and a deranged lipid profile. An EKG and/or a stress test can be advised in this regard.
If the cardiac workup is normal/clear then GERD/dyspepsia is the most likely cause in such cases. PPIs with pro kinetics can be advised for 2 weeks and the symptoms will resolve. The back and chest should be examined first to look for any lesion or dermatomal involvement.
