Question : My scrotum has been burning for a year and now the burning is in the shaft of my penis. I have herpes but I show no sign of an outbreak. In fact I have not had an outbreak in five years. My dermatologist prescribe nystatin/calamine which does work. What do suggest I do

Is there only burning sensation or redness too?
Herpes can be a reason. Sometimes herpes presents without any visible signs and symptoms. But before I arrive at this conclusion, I would like to examine for subtle clinical signs. There is nothing specific evident in the picture.
Apply a combination of calamine and aloevera lotion.
It is a good soothing agent and will relieve burning sensation.
Also take Vitamin B complex capsule daily for 3 months,
These help in nerve regeneration.
Till the time you dont develop herpes lesions, there is no indication to take anti viral medications.

It is an anti viral and prolonged usage can have side effects.
