What Causes Burning Sensation In Stomach And Increasing Redness Around Injury?

I had a scalding injury (caused by momentary contact with a hot Utencil removed from an Induction Stove) on my stomach. Burning sensation was not so severe, and the effect remained at the surface.
Immediately, I cleaned it with lots of cold water, and for about an our applied cold milk packet to it. Within an hour or so, blisters developed. Doctor prescribed "BETNOVATE". But, after taking batch, I dressed the wound with SAVLON and applied SILVEREX cream. After two days, the blisters opened. I continued the same treatment. Yesterday, I noticed slight readness around the burn area, and some whiteness/light yellowishness at one location. I suspected Silverex, and applied SOFRAMYCIN. Overnight the redness around the injury has widened and burning sensation also increased. Please advise..

Hi,

Thank you for your query. I can understand your concerns.
You have developed secondary infection of your burn wound caused by hot utencil. Your first aid of application cold water is correct. However you should take Inj. Tetanus toxoid booster dose,if not taken already. In addition to local application of ointment like mupirocin topical,you have to take systemic antibiotic/s preferably Cefixime (cephalosporin) and dicloxacillin combination to cover especially staphylococcus infection and other organisms,if any.
You also need to see a doctor to examine the depth of wound(superficial or deep burn).

Thank you Doctor. I wish to know why Secondary Infection developed, though I tried all precautions. What could have contributed.!?

(a) I took normal bath, with soap, but followed it up with cleaning with SAVLON.
(b) When the wound (blister) was dry, I applied SILVEREX / SOFRAMYCIN.
(c) When there was oozing, I applied Neosporin powder in the beginning but then switched to Betadine powder.
(d) I left the wound open (or, without bandage etc) even when I dressed up (usually in Cotton T-Shirts/Banians in Hosiery material).
(e) Did not follow any specific dietary restrictions.

I would very much appreciate, if you can spare a little time to examine the above and clarify why I was not able to prevent Secondary Infection. This will definitely educate us on the Do's n Don'ts in case of such injuries.

Hi,

Thank you for your query. I can understand your concerns.
In case of burn secondary infection is often inevitable in spite of taking all precautions. However at home what local management you have done yourself is full of loopholes. For example
Taking bath contaminated the wound with water containing various organisms and also from adjacent skin-water dripped to the wound area.Do not wet the area involved.
You have not used sterile gloves or sterile dressing gauze while applying ointment.Wash your hands thoroughly with soap & water ,dry with sterile gauze and then clean with self evaporating spirits or sterilite solution,if you don't have any sterile glove at hand,before dressing burn wound.
You have kept the wound open ,hence brushing with your garments (certainly not sterile gown)-source of contamination.One can keep a burn wound open but it has to be covered with sterile mesh.
However your burn is only about !%,does not look deep except probably at one or two places and your wound will heal soon with local and systemic antibiotics.

For your information,in any burn unit of hospital, stringent aseptic precaution is taken like as followed in an operation theater and all medical and paramedical staffs working there have to change to sterile dressing (gown,masks etc) each time they visit the Burn unit.
