To recapitulate: Male/69 - Hernioplasty with mesh 38 years ago - wants to know the cause of a burning sensation in the operated area ...
The most probable causes I can think of with the short history you have given are:
- Nerve stretching or entrapment
- Recurrence and part of it pressing on the nerve.

I would advise you the following:
Self examination: Please put a palm on the hernia site and cough forcefully and see if something touches your palm. This is called impulse on coughing and is classical of hernia.
Get a confirmation by getting examined by a General Surgeon and may be a high resolution ultrasound and other imaging examinations as advised by him.

If this is not positive, then other causes can be thought about like any disc problem in the spine, local muscle or fibrosis inflammation and so on.
