What Causes Burning Sensation In Urethra After Protected Sexual Activity?

Question: I went to a prostitute in July 2012 in Korea. It was unprotected oral for brief time followed by protected oral and protected vaginal. Ever since I am having urethra symptoms ie constant burning. I feel there is a wound or sore inside my urethra. When I shake my legs or walk it pricks. Its is below the head of the penis. 10 days ago urine culture is negative. From July 2012, I did not have sex fearing I may infect my wife due to my constant symptoms. This is my first and sex outside my marriage.Blood test for HSV 1 IgG is positive and HSV 2 IgG negative. PCR Swab test for HSV1 and HSV 2 last negative. All other STDs HIV, VDRL, Gonorrhea, Chlamydia etc are negative. Urologists in Korea said I am normal. But my symptoms is not making me normarl. Urologist gave me some medicine gave lot of relief but my stools were black. I stopped those medicines. I dont know what to do.

I understand your concern and situation.

Firstly I would like to tell you that you are not contagious or having STD as per reports. It appears that you are having urinary tract infection. It is possible that sometimes culture could be negative. I would suggest that you must repeat the urine routine microscopy and culture sensitivity for aerobic and anaerobic bacterial infection. The tests must be performed on both initial and midstream sample which must be early morning for three consecutive days. You can get urine analysis also done. Get blood urea and creatinine done. Drink plenty of water and fluids. Practice barrier methods during intercourse in future.

I will undergo tests as per your reply when I go back to XXXXXXX
Please let me know:
1. Is UTI dangerous? Can it infect kidney in my case since the bacteria is unidentified and is like a undiagnoised and thereby untreated.
2. Why is the sharp prick like feeling inside urethra some times when I walk or shake my leg?
3. HSV 1 IgG is positive. Could it be the cause.
4. Will there be any damage to urethra by masturbation/sex and will it be benefit if I avoid them.

I would like to tell you that UTI is not at all dangerous if properly treated with antibiotics for correct duration and follow up. Do not worry about kidney infection. There are other symptoms which are not present in you. Therefore I suggest you to get an ultrasound abdomen done. Yes urethritis can cause prickly pain in perineum. The symptoms will subside on treatment. No HSV 1 does not cause such symptoms so do not worry it causes oral mucosal problems. Yes damage to urethra can be lessened by refraining from sexual intercourse.

Please let me know how is yeast and fungal infection diagnosed. What are the tests assuming they are the cause.

Yeast infections are diagnosed by fungal elements by microscopy and culture for fungi after swab collection. It may take longer in culture.

I think I am not getting proper treatment in Korea. Doctors here are say I don't have any infection. Every time I go they take urine sample and it comes out clear. After I requested them to do urine culture they did and the same report I attached. I gave the urine sample for culture on Monday and by Wednesday (after 2 days) I got the report. I feel it takes longer time to get culture report. This is unusually fast. Hence I am not getting antibiotics precription. Since I am employed here and if the symptoms continues without me taking antibiotics (because doctor does not give)what will happen?

I realize your problem. See for culture sensitivity reports it may take 48-72 hours depending upon the infection. But it has been seen that sometimes the culture does not shows growth despite infection. Antibiotic for urinary tract infection like nitrofurantoin 200 mg twice daily can be taken safely without side effects for 10-14 days. But usually physician guidance is beneficial for follow up. Drink plenty of water. Take an ultrasound to see effects on kidneys usually complications are not so swift.
