Brief Answer:
Anal fissure with Dermatitis.

Detailed Answer:
HI.
Thanks for your query and a clear picture.

The bleeding and burning after passing stool is suggestive of a condition called ''fissure-in-ano''. The picture shows a sentinel pile at 12 O'clock position which comes only in cases of fissure.

Secondly the picture also shows the changes of dermatitis. The cause of both the conditions can be colitis, the infection getting spread to the anal verge to cause the fissure-in-ano and Skin changes as sen in the picture. It also shows the dermatitis spreading backwards to the skin-fold.

I would suggest you the following:
-Laxative to have soft stools.
-Sitz bath in a tub of warm water. Tap dry with a cotton cloth or Paper. Keep the area dry and exposed to air wherever is possible.

-Clean the area with plain water as frequently as possible.
-Apply liquid Povidone Iodine locally to the whole area and allow it to dry.
No other ointments or lotions/ soaps / chemicals are needed.
-A course of an antibiotic and Metronidazole for about 5 days ( you need a prescription please- of a local Doctor)
-This is basically a disorder due to deficiency of vitamins and minerals that the integrity of the skin is lost. So you have to have therapeutic dosage of Vitamin A and D and to continue as multivitamins which are available as anti-oxidant medicines for about a month or so.
_ we have seen the best results in almost all patients with this therapy- thanks again for the photograph for correlation.

You can certainly get an opinion of a Gastroenterologist for a physical check-up and get a prescription of medicines.
