According to the description provided, you seem to be having either of the following conditions which has to be examined and confirmed with investigations.

1. Eustachian tube catarrh is the commonest condition which gives the feeling of fluid sensation in the ear and more so while lying down (due to increased congestion in the supine position). Eustachian tube is a connection between nose and the ear which maintains the pressure between the atmosphere and the middle ear. Blockage of the nasal end of this tube can vary the middle ear pressure giving the sensation of fluid in the ear.

2. Secondly, a blood vessel may be close to the eardrum or a vein similar to a varicose vein may make enough noise to be heard.

3. Other possibilities include dehiscence of the jugular bulb, an enlarged jugular bulb on the involved side is common in persons with venous type pulsatile sensations. This is more pronounced in the supine position due to increased blood flow towards the heard.

4. Finally, the pulsations over the forehead may not be related to the ear. The possibility of stress induced headache or migraine cannot be ruled out.

All the above mentioned are the most probable causes, this requires a thorough evaluation by an apt ENT Specialist.

My sincere apologies, if you feel I have deviated from the actual query. I do understand and respect your concern. But, most of the patients who present to ER with the complaints of buzzing in the head will actually turn out to be having tinnitus secondary to middle ear causes.

There are various other conditions which can also cause buzzing in the head such as anxiety and vascular related disorders in the brain. I avoided quoting these in the initial response, because, I did not want to confuse you and create unnecessary apprehension.

In many individuals having anxiety related issues, they seldom complain of buzzing in the head, which, they actually experience as weird vibrations or tremors in the in the body. There would be no underlying explainable cause for this and usually treated with anti-anxiety medicines.

Other causes related to the brain such as arterio-venous malformations, small intracranial aneurysms and cavernous carotid fistula can give rise to buzzing in the head. This has to be confirmed with appropriate investigations such as MRI and MR angiogram of the brain.

My sincere advice to you is to get a complete examination done by a team of Neurotologist and Neurologists to diagnose the problem and plan the management accordingly.
