Hi
You seem to have developed candidal vulvitis which is basically a fungal infection of the vulva. If you can upload a photo of the rash I would be better able to help.
It is very common in diabetics with uncontrolled blood sugar levels. So most importantly, get your blood glucose levels regularly checked. Since nystatin is not very effective in your case ,you can start application of Canestan cream over the affected areas twice a day. Use it regularly for atleast two weeks .
