What Causes Chest Pain, Muscle Pain In Leg And Fatigue Post Intercourse?

Question: Hi
I've explored different lifestyle in last two years, I have went through clothing optional resorts and swingers club (no sex at all, just went to clubs for no reason)...Due to deprivation of intimate life, I started going to massage parlors and recently I've had a unprotected sex with female CSW in asia (received anal prostate massage unprotected, nude body rub, licked her bottom) and later found out that most of the people in there are +ve. After about 2-3 weeks of exposure I have seen following symptoms in my body which concerns me greatest as I've NEVER felt this type of things before ever with my body.
From May 1 to May 15th or so
========================
1) Night sweats for 2 weeks where I needed to change cloths every time - this is not happening any more.
2) Flacky skin where I can't even properly shave.
3) burning skin even day/night during the same time. I never felt those type of burning skin ever...
4) crazy chest pain where I felt i'll almost die with heart attack
5) too much pain underneath armpits during the same time. Now too much wet armpit with bed smell
6) tonsillitis (white tonsil stones) in my mouth that just pops out every other day now
7) too much leg muscle pain everyday specially from knees to ankle in legs
8) underneath of nails has been turned with white dots with almost all nails. I used to have very shiny nails.
9) few times I had unexplained diarrhea
10) I feel like I should have fever but when you put thermometer, it only goes upto 99.
11) I don't feel like eating anything (loss of appetite)
12) I get almost tired by noon where I used to work tirelessly without any issues
13) can't concentrate on anything
14) metallic taste in my mouth
15) anemia type of symptoms
I've started taking B12 and Iron supplements as well due to anemia type of symptoms My doc initially gave me anxiety (lorazepam .5mg) and I've been taking that from last 20 days so I think if these are related to anxiety it should work...
It feels like to me that I've just seroconverted and now will be screwed up soon ....
NOW, most of those symptoms are gone from last one week so I would say that all these were there for about 2 weeks, that's a sign of worry for me as there's only 2-3 weeks of conversion period.

Now, These are the major symptoms that I have (I won't describe other small symptoms) started from last week or so ago while other are gone (again worried because of conversion window period)
1) So much leg pain in knee and muscle. I can only feel better in morning and by the noon, I'm not me again. (Fatigue and tiredness even after full healthy nurtitious food throughout the day).
2) I have started getting rashes on head (I found out after hair cut) and on my face that people can visually see if they are closure. If I shave, people would definitely see it right away.
3) From last two days I'm getting headaches for no reason, and CONSTANT RUMBLING sounds in my stomach all day even with I eat twice the food that I used to eat, I mean literally twice...2 protein shake, 2 fruit/veg smoothie tall glass, lunch, dinner, yogurt, banana, milk cereal by bedtime...
4) I'm getting loose stool from last month or so....
5) Red rash type of spots on my scalp and little dots on face....
So After exposure in Mid April (~15 April)
1) May 9th - I had 4th gen -ve (Window period?)
2) May 31 - Oraquick home test -ve (manufacture requires 3 month wait time to get conclusive)
3) XXXXXXX 1 - uni-gold recombigen -ve (not sure if this would be good or not)
Based on the info I gathered from following graph, it seems like 95% would test positive with 4th gen IF they get it done at 4th week and no earlier then that... mine was done on 2 weeks so I guess it was just for piece of mind but DOES NOT MEAN THAT IT WOULD BE ENCOURAGING.
http://i-base.info/guides/testing/what-is-the-window-period
aAlso, Based on the info I've gathered uni-gold recombigen -ve , that was done at 5 week which matches with the following graph but the sensitivity is only less then 50%.
http://www.uphs.upenn.edu/bugdrug/antibiotic_manual/HIVtesting-newmethods.htm
I could not find any other disease with all this symptoms... starting from May 1 to getting peak between May 6 to 2 weeks ... getting better suddenly and having this small systems which persist.... :-(
Q1) Are those ARS symptoms
Q2) How sure the above tests are ?
Q3) Would these signs anything other then HIV STD ? Any other curable STI?

Brief Answer:
The possibility of HIV is very unlikely.

Detailed Answer:
Dear XXXXX,

Welcome to HCM. I have gone through your query and understand the concern you are worried about. Being your physician try to solve them at the earliest.

Most of your symptoms subside since one week but some of them remain and they might be due to the muscle spasm or other viral or bacterial infection. Skin rashes can be due to allergy or fungal infection but all these are not due to ARS. Consult your physician or dermatologist for further management.

You had an exposure in mid April and done 4th generation test on 9th May three weeks after the exposure and oraquick home test on on 31st May 6 weeks after the exposure. Both of them are considered highly reliable at given point of time. Alleviate the fear or anxiety related to HIV because the risk is very unlikely.

It is also true that you need to get tested again after 3 months as per routine guideline because the window period for HIV varies from 15 days to 3 months. Avoid thinking too much in that direction or searching many articles on internet because it will only create negative ideas and increase your panic or stress.

As your friend I will also advice to always practice safe sex and stay away from such unsafe or risky sexual activities to prevent the future risk of HIV or other STDs. Do meditation or yoga for mental relaxation and take healthy nutritious diet daily. Forget about the incidence and move on in life with your family and friends. If still any doubt left in your mind feel free to ask me any time.

Thank You

Hi Dr. XXXXXXX

Thanks you for your answer. The thing is all these symptoms started right exactly after 2-3 weeks and were there for 2 weeks without having anxiety.

If I look at the graph below : http://i-base.info/guides/testing/what-is-the-window-period , then 4th gen test at 2 week would be not good unless I'm in that first 10%.

I also had UNIGOLD rapid hiv blood test on XXXXXXX 1st, but based on following it's only accurate 50% : bar chart on http://www.uphs.upenn.edu/bugdrug/antibiotic_manual/HIVtesting-newmethods.htm

Brief Answer:
The symptoms are not related to HIV.

Detailed Answer:
Dear XXXX,

Welcome back and thanks for follow-up.

I totally agree that 4th gen test at 2 weeks is not that much reliable but after two weeks it is better indicator to positivism for diagnosing HIV infection as seen in the next graph because it will also detect P24 Ag along with HIV Abs.

Unigold test is also less accurate but oraquick antibody test is highly reliable after 6 weeks and gives good indication of HIV status so don't get confused or stressed because the risk is absolutely remote. Avoid or forget all what had happened and get tested again after 3 months to get 100% conclusive result.

Thank You

keeping my finger crossed for life changing event.....how much does treatment cost in XXXXXXX per year including almost everything medicine, fees, tests...etc..just general #. . do they have any better medication then viraday for reasonable price?

Answered by Dr. Sunil Jangid (14 minutes later)
Brief Answer:
The treatment is free of cost.

Detailed Answer:
Dear XXXX,

Welcome back nce again.

As you said rightly that 4th gen ELISA is 95% accurate after 6 weeks then why are you thinking so negative. It is just the fear or anxiety of HIV causing all these and bothering you nothing else so calm down and get your self relaxed.

The treatment of HIV is available free of cost at all Anti Retro-viral Treatment Center of the government hospitals but don't worry you need not to have these because it is now very unlikely to get positive result after 3 months of exposure.

All the medicines available in government institutes are very good as like other commercially available like viraday. Stop thinking vigorously about HIV as well as STDs and go for repeat testing after 3 months to get 100% conclusive result.

Thank You

Think positive as you are at very remote risk.

Dr. XXXXXXX Thanks for your time on this one. I'll definitely send you whatever results that I get from 6 week of 4th gen. I really appreciate your information on this one. I'll keep in touch.

Dr. XXXXXXX One last question , I know it must be night time... Could all these be related to any other STD ? Other then HIV ? As long as it's treatable and curable it reduces my anxiety and stress....

Answered by Dr. Sunil Jangid (7 hours later)
Brief Answer:
The possibility is very less likely.

Detailed Answer:
Dear XXXX,

The symptoms you have are non specific for any other STD so the possibility is very less likely and treatment options are available but not needed for you because the result after 6 weeks or 3 months will surely come in your favour so be relaxed and hope for the best. Always stay happy and healthy.
