Brief Answer:
Hello

Detailed Answer:
Hello,
Good day.
100 units of insulin a day is a huge dose. How is his glucose control? What other medicines are used ?

Ear infection in diabetes can go wrong if not adequately treated. If he is gertting chills etc, he has an active infection and Sulfamethoxazole alone may not be sufficient. You must meet an ENT ( otolaryngologist) doctor who can treat your husband well.

Can you send the photo of the ear? Is there any pus discharge? Any greenisg pus? Any blackish dscolouration of outside of ear?

Sometimes, diabetes patients can get a severe form of ear infection and stronger antibiotics like Ciprofloxacin or Augmentin etc may be required to eradicate the infection. Somepeople need IV antibiotics as injections.
We may have to take cultures etc.
His diabetes has to be controlled well as well. Please take him to an ENT doctor soon so that a correct diagnosis can be made.

I wish him speedy recovery
Regards
Binu
