Posted on Thu, 28 Jul 2016
Question: My wife feels like choking and the attack is abrupt and sudden. She then needs to take deriphyllin (injection/tablet) which helps.
We have done Laryngoscopy, PFT, chest X-ray, blood test and everything comes normal.
We are unable to ascertain the cause of the sudden attack.

Brief Answer:
She might be having sudden allergic bronchospasm.

Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
Since she is having sudden attacks, possibility of allergic bronchospasm is more.
She might be having allergy to some dust or smell or food item which is causing bronchospasm and this will cause symptom of choking.
She is improving with deriphyllin. This also supports bronchospasm.
PFT if done during the attack will definitely show obstructive Airway defect. She might have done PFT when choking sensations are not there. This is the reason for normal PFT.
Hope I have solved your query. I will be happy to help you further. Wishing good health to your wife. Thanks.

Even after taking deriphylin tablet the choking sensation lasts for atleast 30 minutes and then there is always a chance that it might reoccur. Also after this she feels weakness and it takes almost 2 days to recover completely.

Hi Doc,
Thanks for your reply.

1. She don't have wheezing while choking. She just tries hard to catch her breath and that's the only sound.
2. PFT report attached.

It's been more than an year, this problem persists intermittently, but we have failed to recognize the trigger. Also, we don't see any pattern like - day/night, seasons, indoor/outdoor etc.

Thanks
XXXX

Now I can see the PFT report. This report is absolutely normal.
In my opinion, we should go for detailed allergy testing to identify the causative allergen in her case.
