Question: Symptoms: chronic stomach bloated and feeling nauseaus

Background:
In 2010 I had my appendix removed.

In 2013 I was diagnosed with GERD and hiatal hernia. I treated it with lansoprazole and the treatment worked. I stopped treatment after I felt cured.

The sensation that I feel now is not heartburn, it's more like a gnawing sensation in the upper abdomen, also there are episodes where I feel bloated even when drinking water. There have been several ocassions where I feel nauseous/dizzy as well. I've been with these symptoms for the past 3 months, but lately it has become worse and is affecting my daily life.

Six months I did some tests for another thing unrelated and everything was nominal: full body mri, xray, blood tests, and urine. (Just a little dehydtrated and had lot of gas that showed in a back xray)

I am currently traveling and I won't be home in at least 4 more months. What would be a recommended course of treatment, should I cancel my trip and go back home? How do I know if its a stomach ulcer?

i read your history thoroughly and understood your concern.

As you mentioned that current symptoms are being experienced since last "three months" and "lately have worsened" , so prior tp traveling; i advise you to :
1. undergo upper GI endoscopic examination
2. have Urea breath test to rule out H.pylori infection

H.pylori is a common cause of ulcer in stomach and duodenum and ulcers can only be observed through "endoscopic examination.

Meanwhile, your advised to take nexium - OTC available drug.
Avoid eating spicy food, junk food and drinking beverages.
avoid brussels sprouts, broccoli, and cabbage as their breakdown in intestine produces gas

NOT an emergency but the early the better.

in case,you have blood vomiting OR black stool OR severe intolerable pain, then you may immediately visit a gastroenterologist.

Meanwhile, as mentioned previously; your advised to take pantoprazole( in chinese 泮托拉唑) - OTC available drug.
Avoid eating spicy food, junk food and drinking beverages.
avoid brussels sprouts, broccoli, and cabbage as their breakdown in intestine produces gas

I am currently in Yangshuo, a small county in the City of Guilin, in the state/province of Guanxi. The only two big capitals close to me are Nanning and Guangzhou, and getting to them the takes 5 hours by car, 2-3hrs by train. I doubt that these provinces have the capabilities to perform emergency procedures, but specially GI emergency procedures, that's my main concern. What do you think?

I don't know if I didn't communicated this, but I've also been experiencing dry mouth, red eyes. Maybe it's unrelated. Also, my gerd/hiatal hernia was discovered after an upper GI endoscopic examination, but it was in 2014. At that time no ulcer was discovered, could an ulcer develop in this time? Should I do a follow up examination?

I started treatment with Omeprasole today (only 1 available here), which I take in the morning. However, I did not feel alleviated, and am still feeling bloated all the time and a bit nauseaus/dizzy. My upper abdomen feels like a knot and is a little tender to touch. All of these symptoms are felt the most when standing. I don't have diarreah, but I have gas.

I've cut irritants from my diet two days ago, but I don't experience my body responding to treatment and am getting little anxious.

Thanks for getting back,

Ulcer is unlikely but you may have developed esophagitis with a history of GERD and hernia. It maybe confirmed or excluded via an endoscopic examination.

I think Yangshuo ( but definitely in GuiLin city) ; there must be well equipped hospitals - and may have endoscopic facilities available. You may approach a hospital and inquire about it. Chinese of upper GI endoscopy is 胃镜 ( weiJing).

Secondly, dry mouth and/or red eyes are not related to the reflux disease.
You may visit a nearby health practitioner/ healthcare center for proper examination and advise.

Thirdly, besides the advise in my first reply, also mix few lemon drops in warm water and drink on empty stomach as well as during the day. Hope it will help to subside the bloating.

I went to the hospital and they performed ultrasound, cbc, and xray. Almost everything was nominal. Although communication of symptoms was a challenge, the Dr prescribed omeprazole and bismuth aluminate (which I've never had before). I attach the CBC to see if you can identify any anomaly. Why is the red blood cell and hemoglobi count high?

1. omeprazole and bismuth aluminate will help to provide releif from the discomfort and bloating.
bismuth aluminate is antacid and intestinal adsorbent.

2. i could not find any blood report attached.
however, mild elevation of hemoglobin or rbc should not be concerning.
