Brief Answer:
Dermatophytosis(Tinea corporis)

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.

It seems that you are having Dermatophytosis(Tinea corporis).Typical infections have an annular/circular appearance ,often referred to as "ringworm."It is characterized by variable erythema, edema, scaling, pruritus and affects relatively hairless skin of the body (back, shoulders in your case).
The diagnosis of tinea can be made from skin scrapings by direct microscopic examination with potassium hydroxide (KOH).
Topical imidazoles(the class of azole antifungals, which includes ketoconazole, miconazole, and clotrimazole) are effective therapies for dermatophyte infections and are generally applied twice daily, and treatment should continue 1 week beyond clinical resolution of the infection.
