The most common cause of cloudy urine is phosphate getting precipitated in it. Phosphates usually come form breakdown of foods and tissues. Precipitation of phosphate in urine may result in a type of kidney stone.
Second common cause is infection of the urinary tract. Infection can be caused by many factors like stagnancy of urine due to some obstruction, stones, etc.

A urinalysis is needed to look for causes of cloudy urine.
