This kinds of weird rash could be allergic urticaria or dermatitis but can be exactly said only after careful visual inspection at the lesion.
It would be better if you could upload the clinical picture of that rashes as there is a feature to upload the images and reports available on the right side this query page.
It would be advisable to see dermatologist or some local GP who can inspect the rashes and can make a diagnosis clear.
Treatment will be started after the final conclusion.

The rash started with one spot looking like a bug bite about 5 days ago and has spread so I don't think its hives. I have some dermatitis on my face only thus the cream I applied. The rash has spread now with about 8 red spots which with inspection some have white heads. The area under the skin has become sore which concerns me. The lowest red area has what seems to be a knot forming below the skin.

This kind of dermatological rashes,a doctor needs to inspect at the lesion in order to make a diagnosis.
From description, it will be difficult to arrive at some conclusion.So,I have asked to take clinical picture and upload as there is a option available to send the reports on the right side of query page.
Now,considering sore skin it might be cellulitis also but need to get examined once.
It would be advisable to see some local doctor or GP who will inspect it and prescribe accordingly.
