Brief Answer:
Can occur in a number of conditions mentioned below.

Detailed Answer:
Hello,

Welcome to HCM and thanks for your query.
Coarse liver can be due to a number causes
conditions that cause hepatic fibrosis
cirrhosis
haemochromatosis
various types of hepatitis
particularly chronic hepatitis
conditions that cause cholestasis
choledochal cysts: probably from secondary cholestasis
certain bile duct tumours: will also usually show of accompanying biliary duct dilatation
diffusely infiltrating hepatic metastases
hepatic lymphoproliferative diseases
hepatic lymphoma: although generally presents as a focal liver disease
hepatic leukaemia
small multiple biliary hamartomas
as a hepatic manifestation of sickle cell disease.
metabolic abnormalities
hepatic amyloidosis
Clubbing of fingers also can occur in a number of conditions mainly of the lungs and the liver. If you have any symptoms these have to be investigated.
The conditions where both occur are
Cirrhosis, especially in primary biliary cirrhosis
Hepatopulmonary syndrome, a complication of cirrhosis

Take care

Answered by Dr. Noble Zachariah (3 hours later)
Brief Answer:
Would probably return to normal after you stopped alcohol

Detailed Answer:
Since your liver size is normal, there are no nodule and the liver functions are normal, cirrhosis has not set in and with the stoppage of alcohol, the liver is very likely to become normal.
I only mentioned the possible causes of a coarse liver and in you probably alcohol is the cause and will become normal with stopping alcohol.
So there is nothing to worry.
I shall be happy to answer your queries any time.
Wish you good health

Answered by Dr. Noble Zachariah (2 hours later)
Brief Answer:
Your liver will return to normal state soon

Detailed Answer:
I would like to congratulate you on your decision to stop drinking alcohol.
The ache you have on the back is not from the liver as the liver inflammation without stretching the capsule is without any pain. You have a musculoskeletal pain and it would be better with a balm or a muscle relaxant.
Don't worry, your liver will become healthy is a short time, now that you have stopped drinking.
Eat wholesome healthy food, exercise at least 30 minutes of moderate exercise at least 5 days a week and practise stress relieving measures like yoga or meditation.
Best wishes,

Answered by Dr. Noble Zachariah (36 minutes later)
Brief Answer:
Remember to keep off alcohol and you will be fine

Detailed Answer:
If you keep off alcohol and do nothing to hurt the liver, it would recover and you have nothing to worry.
You don't have to give up your work because of the coarse liver.
As things improve, your clubbing also will become less and may revert to normal.

Wish you a good day and good health

Answered by Dr. Noble Zachariah (2 hours later)
Brief Answer:
More likely from alcohol withdrawal. It will pass.

Detailed Answer:
Librium is a medicine used to treat anxiety. It also decreases the withdrawal symptoms of alcohol.
If you have shakes, it is more likely from the alcohol withdrawal. You could discuss it with your doctor and he might advise a beta blocker like Inderal if there is no contraindications.
Stick to your resolution to stop drinking. These symptoms will pass.
Best wishes for a good day and good health
