I had Rabies post exposure series Rabavert, and this was my 4th shot yesterday.
1st shot - left, 2nd to right, 3rd to left, 4th to right.

My right hand palm is cold and its lil pale compared to my dark skin. I started to check from injection site, cold area starts from wrist(where we usually check the pulse) and cold all the way to fingertips. And slightly cold right below deltoid injection site.

No pain yet in the arm.

A little swelling at injection site, couldn't notice any color change, but no pain.

Do i need to go to emergency? please advice.

Answered by Dr. Sudhir Kumar (10 minutes later)
Brief Answer:
Need to make sure pulses are well felt.

Detailed Answer:
Thank you for getting back with more details.

We need to make sure that pulses are well felt- radial (at wrist) and brachial (at front of elbow). If they are well felt and if there is no pain, there is no need to worry.

If pulses are weak or not felt, or if there is pain in forearm or wrist, then, it would be better to go to the emergency.

Thank you for the quick reply.

Am scared, what could be worst if there is no pulse? what treatment is sought for this? Please advice what to ask for, once in emergency
doctor
Answered by Dr. Sudhir Kumar (2 minutes later)
Brief Answer:
It can be easily treated with medications.

Detailed Answer:
Thank you for getting back.

There is no need to worry, as even if the pulse is not well felt, it can be treated with medications in most cases.

When you reach emergency, describe your symptoms and the doctor would automatically feel for your radial and brachial artery pulses. If any doubt remains, they would advise you to undergo an ARTERIAL DOPPLER study.

Is there anything to do with auto immune disorder ?

or is this because of injection?

Do i wait for next 24 hrs or go get it checked now?

Please advice.
doctor
Answered by Dr. Sudhir Kumar (1 hour later)
Brief Answer:
You can wait for 24 hours.

Detailed Answer:
Thank you for getting back.

Autoimmune disorder is unlikely as patients with autoimmune disorder have many other symptoms such as joint pains, skin rash, etc.

You can wait for 24 hours, if symptoms are stable.

Sir, i went to emergency, they checked pulse and said its normal and no need for ultrasonic.
Next day morning coldness in hand subsided but weakness in same right hand starting from injection sight to fingers.
Not sure when i will be able to sleep. Thinking of going to uc to get sleeping aid. Also i had random pins and needles all over body but that was a day after 3rd shot lasted a day. After 4th shot on my right hand, stiffness and cold feeling in hand all the way from injection site. Cold feeling went away but now weakness in right hand from injection site to fingers. Do i wait for sleep to occur or take some medication to break the pattern?
Also scared about weakness in hand, is it a nerve damage? Or side effect of the shot? Will it go away? Sir please advice

Brief Answer:
Could be related to the vaccine.

Detailed Answer:
Thank you for getting back.

The symptoms could be related to the rabies vaccine (post exposure prophylaxis). In most cases, there is mild nerve damage and it heals on own over time.

If symptoms of tingling, numbness or weakness remain or increase, you should probably see a neurologist and get further evaluation done (which may include MRI and nerve conduction studies).

If symptoms are mild you may take pregabalin capsules to relieve the tingling/pain. Sleeping aid may be taken if needed for sleep.

Brief Answer:
You can take sleeping aid.

Detailed Answer:
Thank you for getting back.

For sleep disturbance, you can ask your doctor to prescribe a sleeping aid.

For pain in kidney area, ask your doctor to examine and if any doubts remain, an ultrasound scan may be done.

Sir, went to urgent care on saturday afternoon 2:30 pm for not falling asleep and chest pain.
They checked heart rate with ecg, came out normal. Chest pain could be due to acid reflux( made sense bcuz i ate a sub before 20 min)
Then they asked how long was i with no sleep, said 34 hrs.
They prescribed zolpidem 5mg 5 tablets.
Took light food at 8:00pm and went to bed same night at 9:30 pm and took the pill for the first time in my life. Woke up at 7:00 am next morning.

Then today(sunday) drove back to my place(4 hrs drive) completed dinner at 9:00pm, and went to bed at 9:45 pm, took the pill. Woke up at 2:00 AM, since then no sleep. Now its 6:15 AM.
It is bcuz that i took tablet immediately after eating that i couldnt sleep long enough?
Or is it that i grew tolerance in just a night?
Am scared if i can ever fall asleep without any medication.
How long do i need to take this medication?
Can i try to fall sleep by myself ? Or follow taking tablets to fall asleep for 3rd day too?

Or keep myself away from tablets and try to sleep by myself for god knows how long to fall asleep?
If i cant sleep for say 40hrs, will the brain make me sleep at some point at 50hrs or 60 hrs? Do i try this?

Please advice.

Brief Answer:
Zolpidem 5 mg is a light dose of sleeping medicine.

Detailed Answer:
Thank you for getting back.

It is good to note that ECG was normal.

Zolpidem 5 mg is a smaller dose, and the usual dose is 10 mg. So, if you did not fall asleep the second day, it could be because of the lower dose (however, I would like to point out that some people do get good sleep with 5 mg too).

Zolpidem usually does not cause tolerance. Moreover, no sleeping medicine causes tolerance within one day of use.

If you can get natural sleep, nothing like it. However, if you need zolpidem for a few days, there is no problem in taking it.

Best wishes,

Lets say completed 5th dose.

On the 6th day i try to sleep by myself, and never get sleep.

Is it possible that i never get sleep for days continously? At what point do somebody deprived of sleep will eventually fall asleep?

I will be more than happy if i atleast get 2 hours of sleep by myself, atleast some confidence

Brief Answer:
Gradual improvement is expected.

Detailed Answer:
Noted your details.

We expect gradual improvement in the quantity and quality of sleep. Most people are well within 2-3 weeks, however, some can take 6 weeks too.

If any anxiety comes related to the illness including lack of sleep, the problem tends to get aggravated.
