Question : Hello, I have been bearing with a cold for about 5 weeks in which I initially lost my voice but had no cough or phlem, I kept my symptoms under control by taking a symbicort inhaler and a cough syrup made by a local pharmacy with guafenesine, carbixunetil, mucovibrol, celestamine.
After 4 weeks I got some lymph node swelling in the groin area. I went for a check up and the doctor said it was mono. The blood work shows that monocytes and lymphocytes are a little high at 12 and 48, liver enzymes ast 64 alt 100. But the mono test came back igg positive igm negative.
Is this mono or just another cold virus? I haven't had any fever or sore throat.

                                    Brief Answer:
Unlikely to be Mono
Detailed Answer:
HI
Your reports indicate that you had Mono in the past . ( Past means from 1 week to 1 year )
So probably the Cold you had 5 weeks back might have been Mono but I doubt the Lymph node what you have now is due to Mono.
I think the Lymph node is due to some other infection either Viral or bacterial. For the evaluation it's better you get a test called FNAC of the Lymph node .
Hope this information is useful for you .
Any clarifications feel free to ask .
Regards

Hi Doctor, if I had mono 5 weeks ago shouldng the img still be positive? is it possible for the img to be negative and have mono? what are the probabilities. I also have high liver enzymes, could this be due to the cough medicine or the symbicort? or due to the virus even tough it is not a mono virus?
Regards, XXXXXXX

                                    Brief Answer:
IgM wil be positive for 1-2 weeks.
Detailed Answer:
Hi
No IgM will be positive for 1-2 weeks and after that it becomes negative and IgG becomes positive.
IgM testing if done within 1-3 days of infection can show falsely negative results otherwise no other possibilities of having mono with negative IgM.
Liver enzymes are mildly elevated and yes it could be due to some other viral infection. There is no need to be concerned about your liver enzymes. It will normalize in 1-2 weeks.
Any clarifications feel free to ask.
Regards.

Lymph node not due to mono
Detailed Answer:
Hi
What i mean is you had mono 5weeks back and not now. The lymph node enlargement is not due to mono but something else that needs evaluation.
I advise you either wait for a week and if lymph node resolves then its fine else get a FNAC of lymph node or get FNAC straight away.
Lymph node may resolve in 1 week. But if it persists evaluation has to be done.
No medicine is required at present.
Hope this answers your query.
