Tinea Unguium or fungal infection of nails

Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query and after going through the images, my opinion is as follows:

1. It appears to fungal infection of skin, also involving the nails.
2. Local therapy wont help help much and systemic therapy for longer time is necessary.
3. Confirmation can be done by demonstration of fungus through nail clippings via culture, or direct microscopy in KOH demonstration.
4. Systemic terbinafine - 250mg/ day for twelve weeks is the treatment of choice. Also local application of 8% ciclopirox solution, daily will be helpful in controlling it.
5. Keep the foot always dry.
