What Causes Constant High Fever Along With Excessive Sweating?

Question : My husband has been running a fever for over one week. Usually between 101 to 104. It will break and he sweats profusely and temp will be down to 99. He is taking Advil every 3-4 hours. We have been to the ED and urine test, as well as blood test came back fine. They also tested him for the flu which was negative. We were told it was viral and would play out within a couple of days. He is not on any antibiotics because they couldn't pinpoint any specific thing. I am becoming concerned. He is more lethargic and I'm sure getting a little dehydrated. Some ideas would be helpful. I appreciate your concern

Based on your description, further workup is warranted. A thorough physical examination should precede the workup to look for any focus of infection. Enteric fever should be looked for. Stool and blood cultures can be done in this regard. Peripheral smear, chest x ray, CRP , ESR, LDH and basic metabolic panel including liver and renal function tests should also be done as part of the workup. Viral hepatitis profile and an ultrasound of the abdomen and pelvis would also be required.

If the blood counts indicate a viral illness with no predominance of poly neutrophils then the above investigations not be required and symptomatic treatment alone would suffice. If a focus of infection is identified then antibiotics can be advised empirically.
