Question : I wanted to know
why I have constant pain in my lower middle of my back. I can't lay flat on my back it worsens the pain and I can't sit straight up or it also worsens the pain.

According to your history it appears you have a inflammatory condition that gets worse on lying down and sitting position. This could be due to kidney stones or menstrual cramps which tend to get worse on lying down. Muscular low back pain tends to get relieved on lying down. Menstrual cramps can sometimes start with low back pain that can get aggravated on lying down and comes in pauses of a waxing and waning chahacter suggesting a muscular spasm.Kidney stones usually are associated with burning sensation on passing urine and radiation of the pain from the back to the groin.

I advise you to take a pain killer Tablet Diclofenac 75mg twice a day after meal for relief from this pain as it is an anti-inflammatroty medication. You can apply a hot water bottle to the area of pain three times a day. Drink 2 litres of water daily to reduce risk of kidney stones formation. Avoid lifting weights.

You can keep a soft pillow under your lower back while you lie down to not strain your back.

If the pain is not relieved with the above mentioned measures, you can consult your Doctor to rule out the cause of this condition after a thorough clinical examination and necessary investigations like an XRay or Ultrasound, as necessary.

If you seem to be having a sort of constant pain at the lower back then it could be Mechanical back pain. This is back pain due to the muscles going into spasm due to constant straining. Also if you have been recently stressed it could be aggravated especially at night. You must try the hot water bottle application over the painful region, a pillow at the lower back will help. Avoid bending forward and lifting weights. Eat healthy and keep well hydrated. Avoid smoking, alcohol, caffeine and stress. Follow ergonomics at work. You can take pain killer and muscle relaxant combination medication like Tablet Diclofenac with Thiocolchicoside for good relief from the pain. Perform Yoga for stress relief.

I have a friend that told me the pain could be because I have flat feet (my foot doesn't have an arch)

Flat feet usually gives to pain in the feet while walking first. Back pain is usually rare in such cases. You can use medial shoe wedge in your footwear while walking which will provide some support.
