Posted on Tue, 8 Aug 2017
Question: My mom is 85 years old. We started eating dinner around 3 1/2 hours ago and soon after my mom took a few bites she started regurgitating the few bites she ate and the time since she has sporadically continued regurgitating phlegm and saliva. I don't know what to do. She has no stomach pain or esophageal pain. This is clear fluid. No blood.

Brief Answer:
Reflux due to week spincters.(age related). She is ruminating.

Detailed Answer:
Hello,
Thanks for query.

Regurgitation is the spitting up of food from the esophagus or stomach without nausea or forceful contractions of the abdominal muscles. Rumination is regurgitation with no apparent physical cause. A ring-shaped muscle (sphincter) between the stomach and esophagus normally helps prevent regurgitation.A ring of muscle fibers in the lower esophagus prevents swallowed food from moving back up. These muscle fibers are called the lower esophageal sphincter (LES). When this ring of muscle does not close all the way, stomach contents can leak back into the esophagus.
As age increase just like all other organs of body our muscles and sphincters get week.Weekening of these sphincters causes regurgitation of food into food pipe and throat.
As the contents from stomach regurgitates back into esophagus it irritates the soft mucosa layers causing secretion of large amount of mucous and phelgem.

Give her a proton pump inhibitor like Omeprazole 40 mg twice daily along with a prokinetic drug (eg.Domperidone) for 3 days. This will provide you symptomatic relief from symptoms.

Take non spicy plain simple home cooked food for few days.

Follow up after 3 days.An endoscopy can be planned after that.

Thanks.
