What Causes Constant Vaginal Bleeding?

default
Posted on Mon, 9 Nov 2015
Question: Hello, I've been having constant vaginal bleeding for 23 days now. Ultrasound shows endometrial thickness of 19 mm. and a 3cm echogenic cyst on left ovary. Still need to see a gyn but in the meantime bleeding persists. sometimes runny sometimes clumpy and heavy. Does this automatically mean cancer or are there other possibilities for this? What is the likelihood of cancer? I;m 44 yrs old.

Brief Answer:
Can be dysfunctional uterine bleeding.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
You have a thickened endometrium that is responsible for the excessive bleeding that you are having. It is not necessary to be due to cancer. A small echogenic cyst on the ovary can be related to the hormonal imbalance that is causing the hypertrophied endometrium. You need proper examination, a few blood tests and hormonal therapy to control this bleeding.
This bleeding can be DUB and not cancer.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Thank you for your quick response,

I'm sorry, the cyst is actually a COMPLEX cyst containing a few echogenic echoes 31 mm.

What is DUB?

What causes endometrium thickening? I have been reading online that a thick endometrium so high as 19 mm it is highly indicative of uterine cancer?

Brief Answer:
Details below.

Detailed Answer:
Hello.
Thanks for writing again.
A complex cyst is worrisome if it is actually increasing in size. DUB is dysfunctional uterine bleeding. It happens due to a hormonal imbalance. This is also responsible for thickened endometrium that causes an excessive bleeding.
Such thickened endometrium can be associated with cancer but endometrial biopsy is needed first to identify the cause. It is best to consult your gynecologist and get the proper investigations done.
Hope my answer is helpful.

Yes, my cyst has increased in size from 2.3 to 3.1 in thr last 6 months. Why is this worrisome? Please explain.
I got an EMB done today to test for cancer. Pray for me, please. My bleeding is still heavy. I was put on a progesterone to help stop the bleeding. Megestrol Acetate. I am scared of taking it though after reading all the possible side effects.
I think it's just time to get a hysterectomy rather than keep going through scary situations like this. Please advise.

Brief Answer:
You need the progesterone therapy.

Detailed Answer:
An increase in the size of cyst suggests an actively growing cyst that goes in favor of a cancerous mass. You can get serum CA-125 levels evaluated to rule out malignancy.
The progesterone that you have been prescribed is safe and all the side effects are not seen in all the individuals. This will help in stopping the heavy bleeding which can otherwise cause anaemia and further complications.
Hysterectomy is done only when there are specific indications for it. It is a major surgery and is not free of risks and after effects. There has to be a scientific reason before exposing anyone to the risk of major surgery and anaesthesia.
Regards.
