The bacteria in discharge cause dermatitis, hence dermatitis of the affected area and constant wetness.
I would advise you the following:
Stooland blood tests.
Prescription based course of an antibiotic and Metronidazole.
See the effect. If the symptoms improve or not.
If not, may need Colonoscopy, biopsy and further management if needed.
Local cleaning with plain water.
Application of liquid povidone iodine to keep the area sterile.
Add on probiotics and multivitamins for improving the healing.
