What Causes Constipation After Taking Duoluton-L?

Question: Hello doctor. I am 25 years and Unmarried. I have been suffering from constipation for the past six months. Before six months everything was normal until I had this periods problem that my periods last for 2 months continuous. I went to see doctor and she gave me Duoluton-L. After one 15 days course all went right and my periods were normal. But from then onward I have suffering from this constipation problem. Initially I use to go for bowel movement twice a day but from past three months I am suffering from this stool Harding problem.Initially small amoun t of blood used to be there while bowel movement and there was an area which used to swell like small balloon near the ring of anus.I thought it was a pile. But this stool hardening kept going on. Once I had Chronic constipation for few days and I kept on applying pressure.

Few days back I went to see a doctor.He examined me by physical examination and he told me that I don't have piles but I have fissures. He has prescribed me few medicines LOBAK,Piroxicam, Histac 150 and Senasof. He has asked me to have sitz bath and apply an ointment.

I want to know how much time it will take to completely heal these fissures.Also how can I avoid this stool hardening problem. Also I am worried about something else that in the period of this constipation I have urge to urinate frequently though I drink around 3 lt of water per day. Also when I sit I feel pressure in vagina and anus. Is it normal? Or I am suffering from Organ Prolapse? How long constipation can lead to this stage of prolapse? I also feel burning and sensations while I urinate .It feels like swelling inside vagina. Please help me out.Could this be the side effect of Duoluton L?

Dear Ms XXXXXXX

Most common cause of constipation in adults are

1. Decreased intake of fluids
2. Decreased intake of fiber

Constipation is predisposing factor for anal fissure, passage of hard stool leads to stretching of anal opening leading to tear which is fissure. This small ulcer leads to anal spasm causing pain and difficulty in passing stools, hence further worsening constipation. This vicious cycle is to be broken to treat both.

Principle of treatment of anal fissure

1. Ensure stools are soft and well formed: This can be achieved by intake of water, high fiber diet and laxatives

2. Decrease pain : Analgesics and Seitz Bath will achieve it

3. Decrease muscle spasm: This helps in healing . Appropriate muscle relaxant ointment can be used.


After initiation of appropriate therapy healing occurs by 2- 4 weeks. The small mass like thing could be Sentinel Pile ( Lesss likely to be prolapse at your young age, Sentinel pile is a small tag of skin guarding the anal fissure and as the name goes is not true pile). Exact diagnosis will depend on cliniCAL EXAMINATION.

BURNING SENSATION WHILE PASSING URINE: Possibility of Urinary tract infection needs to be ruled out. I would suggest you to get Urine routine and microscopic examination done. The feeling of fullness in vagina is likely to be due to muscle spasm, if there is no mass coming out its unlikely to be vaginal prolapse, especially considering you are young and unmarried. Also none of the symptoms are due to Duoluton.

I would suggest you to send me clinical photograph for further assessment.

I would suggest you the following

1. Plenty of oral fluids: 2-3 liter of water a day
2. High fibre diet: 2 servings of vegetable salad and fruits everyday
3. Oral Laxatives
4. Ointment relaxing anal muscle

Doctor, I am more worried about this prolapse Thing. Whole of my mind is concentrated on this thing that I am feeling heavyness in vagina while sitting. Sometimes when I dont concentrate I dont feel anything. But as soon as I start thinking over it I start feeling that I have such problem. How to check mass coming out of vagina?

While you are straining while passing stools if there is no projection coming out of vagina, its unlikely to be prolapse. I would also suggest a thorough examination from your Gynecologist which will be more reassuring to you.
