Question : I have had a cough with sore throat for 3 days, my doctor gave me a Z-Pak which I started today. I have a pain in my left rib under the breast bone. I was running a fever of 101 but today it is back to normal, should I be concerned about this pain?

I read your question and i understand your concern. Zpak is azithromycin a good antibiotic for respiratory tract and sore throat infections. Continue taking it. Apart from it, warm salt water gargles, warm comforting beverages drink like green tea or herbal tea etc should help.

As far as as pain in epigastric region is concerned that is likely a gastric discomfort and flares up by the use of NSAD advil in your case. Just take some pepto and drink cold milk. Avoid spices and stimulants foods to prevent gastric irritation. Azithromycin on its own also cause gastric discomfort. Wait and see and treat the symptoms.
