Question : Hi my wife is a 60 yr old healthy athlete (runs everyday) and has developed a digestive problem (2 months now) noone can diagnose or help. Symptoms: wakes up at 5:30...runs (when possible)..but by 6:00am has severe cramping and diarrea. This is also lately accomanied by feeling nauseous. During day diarrea and nausea controlable but comes back whenever she eats anything in any amount (we are also vegetarians & very concious and careful about what we eat). In the evenings, she is then constipated with lots of gas and cramping epecially after eating. She has been on an antibiotic...no change...has taken parasite meds and had stool samples checked and they say all is OK as well...no change... We have experimented also with no wheat...no dairy...etc but again...no change...can you help?

I appreciate your concern

The altered bowel habit needs to be worked up. Stool for occult blood should be done. A colonoscopy should also be considered especially if the stool for occult blood is positive, to rule out colorectal carcinoma Stool antigen for H pylori and urease breath testing may also be done. If investigations are satisfactory and no cause is identified, empiric treatment with PPIs like esomeprazole can be started along with mebeverine and sucralfate. Rifaximin can be used to reduce colonic bacteria overgrowth and helps in IBS , too. Ispaghula husk should be added to increase the amount of fiber in the diet.If the GERD/Dyspepsia , abdominal pain persists, 24 hour PH monitoring and upper G.I endoscopy should be considered.
