Thank you for asking HCM.

I have gone through your query and pictures. This is most likely due to recurrent fungal infection and dryness.
For this I would suggest using ointments containing anti fungal agents like miconazole or terbinafine and corticosteroids like Hydrocortisone. These are available in brands like Daktarin or Lamisil.
You can also use natural moisturizers like coconut oil for decreasing the dryness. Using kojic acid based soaps will be helpful to whiten the area also. Brands like kojie san is available for this.

Please suggest a cream to whitening the top area without that black patches!?

One for over come all the black patches and make penis skin tone

I have gone through your pictures. Bumps or dots looks like Fordyce spots only. These are modified sebaceous prominences and are harmless. Taking more garlic and anti oxidants will be helpful to get rid off this.
For clearing fungal infection you can apply terbinafine (lamisil) cream. You can apply kojie San (kojic acid) cream for whitening the skin on that area. For moisturizing , applying coconut oil an hour before bathing will be helpful. If you don't like using it then you can use emollient cream. These all are available in XXXXXXX Lanka.

that if i use terbinafine (lamisil) cream and with that kojie San (kojic acid) cream for whitening (both at one time apply) or one by one?
And use emollient cream
If i use these all how many duration take place to see a better result?

Terbinafine is a highly effective anti fungal agent which has wide uses. It is used in genital fungal infections also. For foot this is given in tablet form orally. So you can use Lamisil (Terbinafine). If this is not available then you can use miconazole. These are over the counter medication which doesn't need prescription. You can use this for twice daily application for 2 to 3 weeks.

After this you can use kojie san cream once daily and emollient cream once daily on separate times like one in the morning and one in the evening. This you should use for long term like 3 months to get proper effect. This is also available with out prescription.

Online prescription is beyond the scope of the online medical forum, please try to understand.

Thnaks for this clear answer doctor since this is paid service i can not active here for three months because i need to contact you with the results after use! because it is sensitive place and it is my penis so can i have you email address please i will only contact if i get a matter while using or after usage if there is not recover my fungal on penis & not gone that patches

I want to mentioning that black patch came there is a scratched happen with my fingers then after there that black patch came

Lamisil is for clearing the possible fungal infections present. Once that is over then you can use whitening cream (koji San cream). This is usually well tolerated when considering other whitening creams. But rarely skin irritation is seen. In such case you should discontinue it.
Moisturizing the skin is also important to whiten and remove scars. So emollient cream or natural moisturizers like coconut oil also should be used as I mentioned before.

Kojie San cream can be used alone also. It has whitening as well as moisturizing agent. So you can start with this and if still dryness is present then you can add emollient cream also. There is no product which is completely devoid of side effects. This one is well tolerated when considering other ones. Kojic acid is the best skin whitening agent even for genital area. Other agents all have long lasting side effects.
Still initially you should use anti fungal creams like lamisil for 2 to 3 weeks before using kojie San.

Finally i came to decision doctor i will by these three and use as you prescribed

However after usage if i am not cured or something happen worst to my genital so i will complain on the procedure about this. So you can make sure about these medicine clearly. i meant these prescribed medicine is directly to my illness/fungal/infections that on my penis NOT A BASIC MEDICINE> IT's for this ill directly.
