What Causes Day 21 Progesterone Level To Be Less Than 1?

default
Posted on Wed, 18 Mar 2015
Question: Please help I have a regular 27-30 day cycle and always have done. I am trying to conceive and in the back of my mind I worry I don't ovulate, hence I went to the doctors and he did a day 21 progesterone, which came back at less than 1, so this means I did not ovulate. I am now on day 33 of my cycle and still have not had a period. I am out of my mind with worry. As the progesterone is so low I don't think I am pregnant but I am worried now my period won't come back. I am also worried that I may be go through Menopause, though I have no symptoms. There is no history of early menopause my mum and her mum were in their 50s. Is pregnancy a possibility?

Brief Answer:
Chances of conception are there.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
The delay in the periods can be due to a hormonal problem or due to the stress and anxiety that you already have. A delay in periods does not mean that you are having an early menopause. Conception is a possibility with proper treatment. You need to get your serum FSH levels and AMH levels evaluated to see the chances of menopause and conception.
You can consult a gynecologist for proper guidance. She can prescribe you hormonal tablets to get a withdrawal bleeding also.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
Regards.

My Day 3 egg reserve was 4.2 taken in October last year so do you see this be a good number and not menopause. I am going to my doctor later to ask for referral to fertility clinic with me be 39. This has scared me that I will never be a Mum. I have done a pregnancy test which is negative but have also been told it could be too early to detect pregnancy by urine

I really hope you can help me. Let me start from the beginning I am 39 years of age, never used any kind of contraception and have never been pregnant. I was never sexually active until I was 26 years of age and then it would only be maybe 3 times a year. We have been more sexually active the past 6 months about twice a week. I have only ever had one partner and that is my current husband.

I got married in July 2012 and we decided we wanted to try for a baby. We have never been consistent with intercourse.

I really want to conceive naturally, do you really see this can happen for us? I am overweight. My husband has 20 cigarettes a day and drinks within the alcoholic guidelines (lager or bitter).

I have seen a Gynaecologist who sees no problems in me conceiving and says I have a good egg reserve for my age. My Day 3 FSH was 4.2. I am going to bed every night crying thinking I will never conceive naturally.

I usually have a regular bleeding pattern 27-30 day cycles. Never have pain or any kind of problem
I can’t afford anything privately it will have to be on NHS my own GP won’t refer me as he thinks I am panicking over nothing.
I have a Clearblue Fertility Monitor, do you see this will help?

1. Is my FSH on day 3 of 4.2 normal?
2. Do you agree the fertility monitor is a good diagnosis for ovulating?
3. Do you see I can conceive naturally?
4. Can I have an infertility check for myself and if so how do I go about this?
5. Any suggestions please to help me achieve what I really want.
6. Am I stressing out too much?

If Clearblue fertility monitor shows 3 bars do I:

1. Ovulate
2. Release an egg
3. Should I conceive naturally
Another thing that confuses me is Fertility Monitor shows Ovulation, but Ovulation strips do not. I think I am getting myself into a Frenzy and should just use the Fertility Monitor and forget ovulation test strips. What are your thoughts please?

Brief Answer:
Details below.

Detailed Answer:
Hello.
Thanks for writing again.
The chances of spontaneous conception are present. But your husband needs to quit smoking and alcohol and you need to increase your frequency of intercourse to at least daily during the fertile days. You can check for your ovulation using LH kit available at all pharmacies. The clearblue monitor will also help.
1. Your FSH is normal.
2. Yes. It helps a lot and is accurate.
3. Yes. You can conceive naturally. Just stop feeling depressed and relax.
4. There is no need right now. Try for spontaneous conception for 6 months and in case it does not help get a referral from your gynecologist.
5. Increase the frequency of intercourse. Decrease caffeine and smoking. Start taking folic acid and Vitamin E supplements.
6. Yes. Decreasing the stress levels will help.
You can use a fertility monitor only and when it shows 3 bars do start having daily intercourse for next 2-3 days. It indicates an ovulation in next 24 hours.
Hope my answer is helpful.
