What Causes Delayed Period In A Woman With PCOD?

you still need to wait for 3 more days...explained
Detailed Answer:
Hello,
Thanks for contacting healthcaremagic.
In a known case of PCOD , If your periods have been delayed and you have ruled out pregnancy by ultrasound and have taken meprate from 14-18 december , the your withdrawal bleed should start anytime between 7-10 days . Its the 7th day today , i suggest you wait for 3 more days and your menses should start by then . Stress itself is a big deterrant in resumption of menses. So patiently wait for few more days.
If the menses dont resume then a D&C may have to be planned to remove the endometrium and then you would have to be placed on low dose oc pills for 6 months to regularize your menses.
So, kindly wait and watch, they should resume ideally.
I hope i have answered your query in detail,
Wishing you good health,
regards

Thank you for your response. My primary reason for worry is whether this will have an effect on my pregnancy. I met two doctors, including one who put me on medications, said no need to worry about pregnancy chances. And other one who said that pregnancy related treatment should be started after periods resume). With these conflicting responses, I am confused.
(i) All my tests regarding TSH, Hb are normal. Random blood sugar is 166, I am little overweight (71 kg, height 5'4"). I have started diet control as well as exercise since last 1 week.
(ii) In the note in ultrasound report (Real time ultrasonography of abdomen and pelvis) - it is normal everywhere in Liver, Pancreas, Gall Bladder, Kidneys, Urinary Bladder, Uterus - no mass lesion, endometrial thickness normal in size and echotexture.
But in the conclusion it is mentioned - IMPRESSION - Polycystic ovarian disease.
With this background, I have following query -
(i) If my period comes within next three days, even then do I need to start any pregnancy related treatment?
(ii) In case it does not come, what should I do - will still D&C be required. Will it have an effect on my pregnancy chances?

Detailed Answer:
hello.
Having gone through the reports , your random blood sugar is high and out of range making you pre diabetic with increased insulin resistance which is associated with PCOS.
nextly you are over weight for your height and age. Obesity has a bearing on ovulation and hence difficulty occurs in conception.
PCOS is categorized by three things, delayed and irregular menses, excessive facial and body hair growth , & multiple cysts in the ovary with an increased ovarian volume more than 10 cc each.
PCOS patients suffer generally from infertility and with insulin resistance , it makes all the more difficult.
If you are contemplating pregnancy then you have to reduce weight, start on anti diabetic treatment after a blood suger fasting /pp test and have to regularize periods first with oc pills for 6 months at least.
You should not plan before 6 months at all as you would put the conception at risk and even if you conceive , with high insulin resistance may have miscarriages or missed abortions.
So , it is advised you seek treatment after menses resume. If menses dont resume on its own by another 3 days and not willing for d&c , then you may wait for it to resume naturally , but there is no fixed predictable time for that. Its better that a d&c is done and treatment started soon .
My concern is your sugar values, this makes you diabetic which has to be controlled first.

Detailed Answer:
hello,
your fasting blood sugar values are high now, that means the previous test was spurious. These are acceptable values but there is an element of diabetes involved.
It is suggested that you may repeat a course pf progesterone challenge again for 5 days if you are not keen on D& C and this time take the following schedule. :-
TAB MEPRATE 10mg TWICE A DAY FOR 5 DAYS.
No other medication with it. Wait for next 5 days , periods withdrawal should start by then.
If not then , D& C would be an option.
