What Causes Development Delay In A Child?

Question: Hello Doctor,

My sister’s 3 year old son has been showing developmental delays right from his birth. This is totally based on my observations of him and his activities over these 3 years. No doctor/paediatrician has yet indicated anything of that sort and I think my sister too hasn’t realised this fact.

At 3 years of age, he
-     Makes no eye contact
-     Doesn’t respond to name; occasionally does it for his mother
-     Ignores existence of people around him
-     No stranger anxiety. Is not shy when guests pick him up.
-     Cannot communicate clearly, only his mother can understand him
-     Doesn’t play with toys like cars, blocks that a normal 3 year old would
-     Has affinity for books and memorizes everything his mother reads out from them. Can recollect the same the next time.
-     He’s good in numbers, alphabets and shapes (can list out tough ones like ellipse, trapezoid, parallelogram..).
-     Sings rhymes, numbers and alphabets without any context throughout the day.
-     Appears to be ‘in his own world’ most of the times;
-     His tantrums are most often too difficult to control
-     Eats only when specific religious songs are played
-     Scared of slides, merry-go-around and play area that a normal 3 year old would be crazy about
-     Doesn’t bother if other children take his toys
-     Repeats anything that we tell him
-     Is hyperactive and keeps walking around the house most of the time when awake
-     Cannot play hide and seek with my 3yr old son

I’ve listed out what I think I’ve seen of him. I am very apprehensive about taking up this topic with my sister. And I am also not 100% sure about the problem seriousness; I am not a doctor, just a mother of another normal 3 year old and can definitely understand the developmental milestones at this age.
Can you please guide me what to do next?

The symptoms you mentioned,

- Makes no eye contact

Above all are suggestive of autism spectrum disorder.

It is very good observation from your side. For confirmation of diagnosis child needs to be evaluated by psychiatrist with detail history from mother. Even some pediatrician can evaluate this aspect also.
Possibility is also there that this is just normal deviation from regular development due to any social/family/parenting factors.
If at all it is related to Autism age is only 3 and early intervention can help a lot.


Other symptoms you mentioned,
- Has affinity for books and memorizes everything his mother reads out from them. Can recollect the same the next time.
- He’s good in numbers, alphabets and shapes (can list out tough ones like ellipse, trapezoid, parallelogram..).

These suggest that child is likely with normal intelligence (IQ) and that is good.

You should not hesitate to discuss this aspect with your sister. You can talk with her in positive manner that whatever you feel that should be checked and if at all something wrong is there we can treat it early and in effective manner so child can adjust at school.
After convincing her first step is to discuss issue with current family pediatrician and then child can be referred to specialist.
