Question: Doc,

I've been having joint pain, lost of appetite, fullness, diarrhea and cramps, also lost about 10 lbs in a week. Went to a local doctor and was given some antibiotics. Doc told me it "might" be a bug or virus. A second opinion would be great. My anxiety isn't helping me much. What can this be linked to?

What is the nature and distribution of the joint pain, exactly what joints are affected. Is there specific joint involvement or generalized body aches ?
Has the diarrhea and cramping persisted for one week
Was there any blood in the stool
Was/is there any fever associated with these symptoms
Were any investigations advised, such as a full blood count, if so then please upload a copy of the labs done

The joint pains are as followed : eblows, risk, and knees, just recenlty some shoulder pain mainly left shoudler, the diarrhea has been persist for a week, no blood in the stool but it is very dark. No fever at all, no blood work was advised

Hello again.

Based on your description, its likely to be reactive arthritis. The antibiotics should be continued and the course should be completed. Isotonic fluid replacement should also be done. If the symptoms persist despite the completion of antibiotics then workup including FBC, ESR/ CRP, stool R/E culture and an ultrasound of the abdomen pelvis should be done in consult with your doctor.

For now, the stool should be examined and a possible lower GI bleed should be ruled out. A stool for routine exam plus ova and parasites can be advised.
