What Causes Diarrhea After Having Food In A Lactose Intolerant Child?

Question: My 5 and 1/2 month baby is having lactose intolerance from a week and due to it she passes loose motion after each feed .around 10-15 times she passes loose motion ,but she is active and also there is a weight gain of abt 100 gms in a week and urine output also good .but my led Dr asked me to stop breastfeeding for couple of days and start zerolac .she is not happy with zerolac and it is very difficult to feed her .is it really require to stop breastfeeding?

Hi.....What your kid is having is not a diarrhea and is only a gastro-colic reflux. It is quite common for babies of this age group t pass small amount of diarrhea or loose stools soon after feeds. This need not be treated as diarrhea and especially antibiotics are not indicated.
Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry.

Hi...Sorry to have not made myself clear.

1. No need to stop breast feeding.
2. No need to switch over the Zerolac.
3. If the baby is gaining weight and passing urine 4-6 times per day, there is no need for nay sort of treatment or change of feed.
4. As I said in my previous interaction - Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting or loosing weight...you need not worry. Non of these are there in your kid. So do not worry at all.
5. WHO - World Health Organization too has mentioned that exclusive breast feeding till 6 months of life is extremely good for babies.
6. I am not able to see any stool report here in my panel. Can you kindly upload it once more and get back please.

Hi..Thanks for writing back.

I have gone through the report. Now I understand why the doctor there felt it is lactose intolerance. The positive report on the stool reducing substances might have prompted him to take the decision on lactose intolerance.

But frankly speaking any normal breast fed baby will have reducing substance in the stool and any laboratory report should be seen in conjunction with the baby's general condition.

The same report with a severely failing to thrive baby, with blood in stools, no urine output, skin allergy and eczema and very lean baby - we might consider the possibility (only possibility - not confirmation) of lactose intolerance.

Before getting this test done ,Dr had given her septran b'coz he had told she is having infection since my kid keeps her hand in her mouth ,so I completed tat course .Now after your confirmation I think I can continue my breastfeeding .

For how long does she passes loose stool like 10-15 a day .

Hi...To be XXXXXXX in answering your question about how long she passes loose stool like 10-15 a day - theoretically gastrocolic reflex is a life long reflex (as per medical literature) and it can last long.

But practically speaking as the babies grow up I have seen the babies coming out of this reflex by 8-12 months of age gradually. Some even come out of it even by 6-8 months.

Septran is not the reason for this stool report and there is no need to use antibiotics in young kids unless specifically indicated and the kid is sick.

PLEASE CONTINUE EXCLUSIVE BREAST FEEDING TILL 6 MONTHS OF AGE.

Hello Dr,

As u told to stop zerolac and continue breastfeeding ,am doing the same from yesterday evening ,yesterday her loose motion reduced to 5 and it WS green watery stool but today the condition is still bad then yesterday she has already passed 4 times by evening 4 but today and yesterday it was not immediately after feed it was after some time of feed say some1/2-1 hr of feeding .And its green and watery .y is it green and if she pass 1 hr aft feeding does it again referred to gastrocolic reflex

Pls help me and suggest me .

From before ,she was a poor feeder but once her loose motion had increased she was feeding well and today again she didn't feed very well like last week when she was passing loose motion very frequently

HI...it will take some time to settle. Do not worry.

As I have already mentioned in my previous interaction - Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry. Green colour motion is acceptable in this age group. Passing stools after 1 hour need not be gastrocolic reflux but if you are bottle feeding it is a quite common occurrence.

You say she is a poor feeder. Please answer the questions below -
Questions:
1. What was the birth weight and what is the current weight?
2. How is the attaining of developmental milestones?
3. Is the stool oily and floats in the pan?
4. Is there any colour change in the hair or skin?
5. Is there any history of recurrent serious infections?
6. Does she vomit every time after feeding?

Her birth weight WS 3 and now she is 7,her stool is not oily and she didn't have any serious infection and she doesn't vomit after feeding and her skin and hair hasn't changed

But I have stopped bottle feeding yesterday itself but still she is passing green loose stool and is it fine if she passes loose motion after 1hr of feeding or is it diarrhoea ?

Hi....

1. Her weight fain is more than enough - nothing to worry. Another issue is that she should be doubling the birth weight by 5-6 months...means it should be 6. But she is 7 kg. So it is a perfect evidence that she is not a poor feeder. At any stage of a baby's growth rather than weight and quantity of feeding - the best indicator if normalcy is her activity.

2. Bottle feeding could have caused this diarrhoea and green stools. I suggest you use zinc supplements (Z&D drops 1ml once daily for 14 days).

3. It is fine if she passes loose motion after 1hr of feeding also provided she doesn't have ay of the danger signs which I have already mentioned. If she is active otherwise - just leave it and she will be fine it another 3-4 days.

1. Zinconia and Z&D drops are same. Do not start Z&D. Continue Zinconia at 5ml per day for total 14 days.

2. Nutolin-B is a probiotic. You can continue it for total 5 days.
