What Causes Diarrhea In A Breast Feeding Infant After Having Antibiotics?

Question : My seven month old exclusively breast-fed baby has had diarrhea for over a month now after receiving antibiotics. We stop anabiotic's on day two because of diarrhea that has not resolved. The diarrhea was very green and watery for the first two weeks it's now returned to its normal yellow color but is still very greasy and he's having multiple bowel movements a day 8+ times. Stool samples were collected two weeks ago and all results came back negative I believe they tested for parasites and C. difficile. They are now sending a second set of cultures out to retest. During the past month my infant has had two ear infections and was diagnosed with RSV which then led into pneumonia

I would like to know whether your baby has lost weight during the illness.
Both diarrhea and respiratory infections are common in this age.But as the diarrhea is persistent for a month it should be evaluated.The prolong use of antibiotic can cause diarrhea 1) by destroying the normal flora of the gut which are very helpful in absorption or 2) due to antibiotic associated secondary infection with C.difficile.
The infection by any parasite or C.difficile has been ruled out already by the stool test.
Due to some mechanism in the gut the stool gets it's normal yellow color. Due to rapid movement of bowel there is little time in the gut to change the color of stool from green to yellow.Following diarrhea sometimes it may take 2-3 weeks for the bowel pattern to become regular.In diarrhea there may be transient intolerance to the lactose and if the baby is breast fed then the loose stool may take more time to recover.
As diarrhea is there for more than 2 weeks I would like to rule out the other causes of diarrhea other than infection.As you have mentioned that the stool is greasy,I will get a stool test done for fat globules to rule out steatorrhea(diificulty in absorption of fat) and for reducing substance(to rule out any lactose intolerance).Although the condition like cystic fibrosis is an uncommon cause of diarrhea,I would like to rule it out as there is past history of pneumonia and ear infection.
If the test comes normal I won't be overly worried.

Thank you for your quick response. We haven't noticed any weight loss him over there has not been any weight gain in the past six weeks. As he is exclusively breast-fed I don't believe there is any lactose issues as I eat a vegan diet. We are still awaiting the stool samples to come back for the second round of testing. However in light of the recent Tylenol recalls is there any way this could have caused the issue. At the initial onset of the ear infection we treated the fever with Tylenol several times. The diarrhea started after that.

Tylenol usually does not causes loose stool

There are various studies and none have shown loose stool as a side effect of tylenol(acetaminophen/paracetamol).
Usually people misinterpret it as a side effect because most of the time tylenol is taken for control of fever(mostly viral) and loose stools are usually due to viral infection.
Although it is usually not advised to stop breast milk if mild lactose intolerance is suspected.But as breast milk also contains adequate amount of lactose,it may need to be discontinued temporarily if the intolerance is severe.
I don't consider it to be a side effect of tylenol.
The stool pattern is improving now(as you have described).It may take few more days to get the bowel pattern regular.
Discuss the need of investigation I have suggested earlier,with your pediatrician.
