Brief Answer:
A Coronary angiogram is a must in his case.

Detailed Answer:
Hi .

I understand your query and concern.

This form of difficulty in breathing could be related to impaired pulmonary ventilation.

This exercise induced breathing difficulty could either be due to a cardiac or a lung cause.

A normal echo will not rule out heart disease.

I advise you to consult a cardiologist for coronary angiography to rule out coronary artery disease.

In case if CAG is normal, you need not worry much.

In such a case simple home oxygen and bronchodilator therapy is all that is enough.

Thank you.
