What Causes Difficulty In Getting Pregnant With Normal Test Results?

Question : Hi Doctor;
My current wife and I have spent almost two years together. Despite our regular attempts we could not make a baby since we have established our first sex in April 2013. My wife and I have visited two different gynecologists and she was taken through comprehensive check ups including blood and hormonal tests, ultrasound and urine analysis. She was put on medication for two to three months. Unfortunately, she couldn't become pregnant. We visited another gynecologic specialist. Same check up has been done and medications have prescribed. Again she couldn't get pregnant. I have also done seminal analysis and all results were normal.
Could you please give the possible problems and to deal with them? What possible steps should I take in order to make her pregnant?

Hello,
Thanks for trusting us with your health concern.
It seems that both of you are healthy but are unable to conceive. The very first thing, you both have to involve in unprotected sex without any contraceptive method around ovulation day or fertile days for at least 6-12 months before proceeding to basic investigations of primary infertility. Ovulation occurs 14 days prior to next period due date. In case of regular period with 28-30 days duration, her fertile days will be from 11th-18th days of each cycle counting the date of start of periods as day 1 of cycle in women with regular cycles. It is recommended to have sex at least on alternate days during these days to have best results.
Lying down on your back a pillow beneath your buttocks is considered the best position. She can use ovulation prediction kit (LH kit) to predict ovulation time in mid cycle. With this protocol, chances of pregnancy can be maximized.
Having sex at night just before sleeping is preferred because female tends to lie down for longer time without any activity and thus assisting in spermatogenesis ascent.
In preconception period, you both can take Vitamin-E and folic acid supplements for at least 3 months. Additionally, take healthy diet, control body weight by regular physical exercise, try to avoid mental stress/ emotional upset by regular meditation/ yoga, drink more water and eat fresh fruits, maintain genital hygiene and take sound sleep with complete relaxation.

Hi Dr Tayal,

Thanks for the detailed information and instructions. Do you recommend single sex per day or multiple sex?
Normally, there is a leak from the vagina which appears after having sex. I am not sure if this is normal. According to the seminal analysis my average volume of sperm is beteeen 2.5 to 3.0 millimeters. Please advise.
Finally, my wife experiences abdominal pain during the period. Is this normal?

Thanks for writing again.
It is best to have sex only once during the day during the fertile period to maintain the sperm quality.
The leakage of semen from vagina after sex is a normal finding. It is not harmful or does not decrease the chances of conception. She can use a pillow to decrease the spillage.
It is the motile and healthy sperms that matter and not the semen volume. Please attach the full report for proper analysis.
The abdominal pain during periods is normal. It is due to uterine contractions and can be controlled with a mild pain killer.

Semen test needs to be repeated.

I have seen the reports attached. Your semen analysis shows only sluggishly moving sperms. There is no comment regarding the sperm morphology. It is best to get the test repeated from a good laboratory where the sample is analysed within few minutes by a qualified pathologist.
Your wife's reports are normal.
